What is Transvaginal Ultrasound?

Transvaginal ultrasound is a specialized imaging technique where a small ultrasound probe is gently inserted into the vagina to obtain detailed images of the pelvic organs. It's one of the first-line imaging tests for suspected endometriosis and can detect certain types of endometriosis lesions.

What Ultrasound Can Detect

  • Ovarian endometriomas (chocolate cysts)
  • Deep infiltrating endometriosis (DIE)
  • Adhesions between organs
  • Endometriosis affecting bladder or bowel
  • Uterine abnormalities (adenomyosis)

Types of Ultrasound

Transvaginal Ultrasound (TVUS)

The most effective type for endometriosis. Provides high-resolution images of pelvic organs from inside the vagina.

  • • More detailed than abdominal ultrasound
  • • Takes 15-20 minutes

Transabdominal Ultrasound

Performed over the abdomen. Less detailed but useful for overall pelvic assessment.

  • • Good for large cysts
  • • May be combined with TVUS
  • • Requires full bladder

Transvaginal Sonography (TVS) with Bowel Preparation

Advanced technique for detecting deep infiltrating endometriosis affecting the bowel.

  • • Requires bowel preparation (enema)
  • • Best for deep endometriosis
  • • Higher accuracy for DIE
  • • Specialized technique

The Procedure

1. Preparation

Empty bladder before transvaginal scan. No special preparation usually needed. Wear comfortable clothing.

2. Positioning

Lie on examination table with knees bent. Similar position to pelvic exam.

3. Scanning

Ultrasound probe covered with protective sheath and gel is gently inserted. Probe moved to capture different angles. May feel slight pressure but should not be painful.

4. Duration

Typically takes 15-30 minutes. Immediate results available. No recovery time needed.

Accuracy & Limitations

High Accuracy For:

  • • Ovarian endometriomas (90-95% sensitivity)
  • • Deep infiltrating endometriosis (80-90%)
  • • Bladder endometriosis
  • • Rectovaginal endometriosis

Limitations:

  • • Cannot detect superficial peritoneal lesions
  • • May miss small implants
  • • Operator-dependent accuracy
  • • Cannot definitively diagnose without biopsy

💡 Important Note

A normal ultrasound does NOT rule out endometriosis. Many women with confirmed endometriosis have normal ultrasound findings, especially those with superficial peritoneal disease. If symptoms persist despite normal ultrasound, further evaluation with MRI or laparoscopy may be needed.
