Tight Foreskin (Phimosis): Treatment in Nepal
Phimosis — the inability to retract the foreskin over the head of the penis — is a common condition that affects men of all ages. It is treatable, and seeking help early prevents complications.
What is Phimosis?
Phimosis occurs when the foreskin is too tight to be pulled back (retracted) over the glans (head) of the penis. It can be:
Physiological phimosis: Normal in infants and young boys. The foreskin naturally loosens with age and usually resolves by puberty.
Pathological phimosis: Develops in older boys or men due to scarring, infection, or skin conditions. Requires treatment.
Symptoms of Phimosis
- Inability to retract the foreskin
- Pain or discomfort during erection
- Pain during sexual intercourse
- Difficulty urinating (in severe cases)
- Recurrent foreskin infections (balanitis)
- Ballooning of the foreskin during urination
Causes
- Recurrent infections (balanitis)
- Lichen sclerosus (a skin condition causing scarring)
- Forceful retraction in childhood causing scarring
- Poor hygiene
- Diabetes (increases infection risk)
Treatment Options
1. Topical Steroid Cream First-line treatment for mild to moderate phimosis. A steroid cream (betamethasone) applied to the tight area twice daily for 4–8 weeks gradually loosens the foreskin. Success rate: 70–90% for mild cases.
2. Stretching Exercises Combined with steroid cream, gentle manual stretching of the foreskin can gradually increase its elasticity. Requires patience and consistency.
3. Preputioplasty A minor surgical procedure that widens the foreskin opening without removing it. Preserves the foreskin while resolving the tightness.
4. Circumcision Surgical removal of the foreskin. The definitive treatment for severe or recurrent phimosis. Performed under local or general anesthesia as a day procedure. Recovery takes 1–2 weeks.
When to Seek Treatment
- If phimosis causes pain during erection or intercourse
- If you have recurrent foreskin infections
- If urination is difficult or painful
- If the foreskin becomes trapped behind the glans (paraphimosis — a medical emergency)
