Weight Gain Due to Thyroid: Causes & Management
Unexplained weight gain is one of the most frustrating symptoms of hypothyroidism. Many women in Nepal struggle with stubborn weight gain for years before discovering their thyroid is the cause.
How Hypothyroidism Causes Weight Gain
The thyroid gland regulates metabolism — the rate at which your body converts food into energy. When the thyroid is underactive (hypothyroidism), metabolism slows significantly, leading to:
- Weight gain even with normal or reduced food intake
- Difficulty losing weight despite diet and exercise
- Fluid retention causing puffiness
- Increased fat storage, particularly around the abdomen
- Fatigue that reduces physical activity
How much weight gain? Most people with hypothyroidism gain 2–5 kg due to the thyroid condition itself. Additional weight gain is often due to reduced activity from fatigue and metabolic changes.
Other Symptoms of Hypothyroidism
Weight gain rarely occurs in isolation. Look for:
- Fatigue and sluggishness
- Cold intolerance
- Constipation
- Dry skin and brittle nails
- Hair thinning or loss
- Depression or low mood
- Brain fog and poor concentration
- Irregular or heavy periods
- Muscle weakness or cramps
Diagnosis
A simple blood test:
- TSH — elevated in hypothyroidism
- Free T4 — low in hypothyroidism
- TPO antibodies — positive in Hashimoto's thyroiditis (autoimmune hypothyroidism)
Treatment
Levothyroxine: Synthetic thyroid hormone replacement. Once TSH is normalized, metabolism improves and weight loss becomes possible with appropriate lifestyle changes.
Important: Thyroid treatment alone may not cause dramatic weight loss. It corrects the metabolic deficit, but additional lifestyle changes are needed.
Weight Management with Hypothyroidism
Once thyroid levels are optimized:
Diet:
- Focus on whole foods, lean protein, vegetables, and complex carbohydrates
- Limit processed foods, refined sugars, and excessive sodium
- Selenium-rich foods (Brazil nuts, fish) support thyroid function
- Adequate iodine (iodized salt, dairy, seafood)
Exercise:
- Regular aerobic exercise (walking, swimming, cycling) — 150 minutes/week
- Strength training to build muscle and boost metabolism
- Consistency is key — results take time
Lifestyle:
- Adequate sleep (7–9 hours) — sleep deprivation worsens thyroid function
- Stress management — chronic stress elevates cortisol, which impairs thyroid function
