What is Watchful Waiting?
Watchful waiting (also called observation or active surveillance) is a management approach where a small, simple ovarian cyst is monitored over time rather than treated with medication or surgery.
Most functional ovarian cysts are harmless and resolve on their own within 2-3 menstrual cycles. Watchful waiting avoids unnecessary interventions while ensuring the cyst is properly monitored.
Key Fact: Up to 70% of functional ovarian cysts resolve spontaneously within 3 months without any treatment.
When is Watchful Waiting Appropriate?
Good Candidates
- • Cyst smaller than 5cm
- • Simple, fluid-filled cyst
- • No or minimal symptoms
- • Premenopausal women
- • Normal CA-125 levels
- • No concerning features on ultrasound
Not Appropriate When
- • Cyst larger than 5cm
- • Complex cyst with solid areas
- • Severe or persistent symptoms
- • Postmenopausal women
- • Elevated CA-125 levels
- • Suspicious features on ultrasound
Monitoring Schedule
Initial Follow-Up (6-8 Weeks)
Repeat ultrasound after 2 menstrual cycles to check if the cyst has resolved. Most functional cysts disappear by this time.
Second Follow-Up (3-6 Months)
If the cyst persists but is stable and simple, another ultrasound in 3 months may be recommended.
Long-Term Monitoring (6-12 Months)
For cysts that persist beyond 6 months without change, annual ultrasound monitoring may be sufficient.
What to Watch For
While on watchful waiting, monitor for these signs that may require re-evaluation:
Seek Immediate Care
Sudden severe pain, fever, vomiting, dizziness, or rapid breathing may indicate cyst rupture or torsion.
Schedule Appointment
Worsening pelvic pain, increasing bloating, changes in periods, or new symptoms developing.
