What is a CA-125 Blood Test?
CA-125 (Cancer Antigen 125) is a protein that can be found in higher than normal levels in the blood of some women with ovarian cancer. The CA-125 blood test measures the amount of this protein in your bloodstream.
While primarily known as a tumor marker, CA-125 is used in ovarian cyst evaluation to help assess the risk of malignancy, particularly in postmenopausal women or those with complex cysts on ultrasound.
Important: CA-125 is not a definitive diagnostic test for ovarian cancer. Many non-cancerous conditions (endometriosis, fibroids, pregnancy, pelvic infection) can also elevate CA-125 levels.
When is CA-125 Recommended?
Postmenopausal Women
Higher risk of ovarian cancer; CA-125 is more useful in this age group.
Complex Cysts on Ultrasound
Cysts with solid components, septations, or irregular walls may need further evaluation.
Family History of Ovarian Cancer
Women with genetic predisposition (BRCA mutations) may benefit from regular monitoring.
Persistent or Growing Cysts
Cysts that do not resolve or continue to grow on follow-up ultrasound.
Understanding CA-125 Results
Normal Levels (Below 35 U/mL)
- • Low risk of malignancy
- • Cyst is likely benign
- • Regular monitoring may suffice
- • Repeat ultrasound in 6-8 weeks
Elevated Levels (Above 35 U/mL)
- • Does not confirm cancer
- • Further tests may be needed
- • MRI or CT scan recommended
- • May need surgical evaluation
Note: CA-125 can be elevated in many benign conditions including endometriosis, uterine fibroids, pelvic inflammatory disease, pregnancy, and even during menstruation. Your doctor will interpret results in context of your age, symptoms, and ultrasound findings.
Other Blood Tests for Ovarian Cyst Evaluation
HE4 (Human Epididymis Protein 4)
Another tumor marker often used alongside CA-125 for improved accuracy. Part of the ROMA (Risk of Ovarian Malignancy Algorithm) score.
Pregnancy Test (hCG)
To rule out ectopic pregnancy, which can present similarly to an ovarian cyst.
Hormone Levels
LH, FSH, estrogen, and testosterone to evaluate hormonal function and conditions like PCOS.
Complete Blood Count (CBC)
To check for infection or anemia if cyst rupture is suspected.
